 Registration Information - Non-physicians


ONLINE REGISTRATION IS NOW CLOSED.

Use the registration form below for all non-physicians (ophthalmic technicians and practice administrators may register together).
Ophthalmologists must use the physician form

  • Early bird (received at ISEPS/COS office on or before February 29) - $125
  • Regular rate (received after February 29) - $150
  • Group Discount
    • ISEPS Practice Members may receive group discounts for non-physician attendees (administrator and ophthalmic tech registrations combined).  In order to receive the discount, you will need to use the paper (PDF) form.
    • 5 - 9 attendees = 5%    10 - 19 attendees = 10%     20+ attendees = 20%

The online registration form will automatically calculate the correct registration fees for the number of attendees.  It also will apply the "Early Bird" rate.  ISEPS Member Practices who wish to receive the group discount (5 or more non-physician attendees) must use the paper (PDF) form.

Refund Policy --  Although public health authorities continue to state that exposure to the coronavirus remains at a relatively low risk factor for most Americans who do not have underlying medical conditions, ISEPS-COS has extended its refund policy.  Any cancellations received by the ISEPS/COS office no later than 5 p.m. on Thursday, March 5th will be eligible for a refund less our standard $25 processing charge.  We are unable to offer refunds for "no shows" at the conference.  If you wish to cancel a registration, please email or call with the name of the attendee and the session for which they registered (physician, administrator, ophthalmic technician).

If a registered attendee is unable to attend, you may substitute somebody else from your office in their place.  Please contact the ISEPS/COS office as soon as you are aware of the substitution so we can make the appropriate changes to our registration system.

Meeting Site Information

The conference takes place in the conference facility of the Donald E. Stephens Convention Center in Rosemont, IL.  The venue is near O'Hare International Airport and is close to major highways and just two blocks away from the CTA's Blue Line.
